Bolster Community is developing a new Connected Communities Hub to improve access to support for older people across Newry, Mourne and South Armagh.
Supported by a three-year grant from Belfast Charitable Society, the project is bringing together statutory, health, voluntary, faith and community organisations to create a more joined-up approach to supporting older people and their families.
Inspired by the Not Done Yet report and the successful Family Support Hub model, the project aims to make it easier for older people, families and professionals to understand what support is available and access the right services. It will also help organisations work more effectively together, reducing the risk of people falling through gaps between existing services.
During the first year, the project is focusing on understanding the support already available across the area and building relationships with key referral partners, including local authorities, GP practices and community organisations. A multi-agency steering group will help shape referral processes and develop the Hub model.
As the Hub develops, referrals will begin to be accepted, with full implementation planned towards the end of the first year.
In its second year, the project will expand its delivery. A dedicated Connected Communities Co-ordinator will help older people and their families navigate available services, make appropriate referrals, follow up on outcomes and coordinate support around individual needs.
The aim is simple: to make it easier for older people to find the right support and for the organisations supporting them to work together effectively.
